A draft of the IPv6 sub-TLA request form is now available on the APNIC web
site at:

We invite you to review this form and we welcome any comments or
criticisms you may have. Please feel free to refer to any aspects of the
form which you feel are important.
We would point out, however, that the scripts controlling the output of
the form have not yet been activated. At this stage, you will see the form
as a continuous page, but when those scripts are activated it will be
served to the user in separate sections that must be completed before
progressing.
Also the Provisional IPv6 Assignment and Allocation Policies document
contains two sets of criteria for sub-TLA allocations, General and
Bootstrap. APNIC has decided to restrict the current form solely to
requests made under the Bootstrap criteria. We will shortly prepare a
separate draft form for requests under the General criteria. Requestors
will be directed to an intermediary web page which will then refer them to
the appropriate form.
We have decided upon this approach in order to reduce the level of
complexity of interface presented to requestors. This approach also has
the advantage of allowing us to simply phase out the Bootstrap form when
required and not be left with redundant "legacy questions" in the
operating form once the bootstrap period is over.
