I think there may be some misunderstandings here. Hopefully I can clarify a few things.

The guidelines document is an informational document and does not contain any policy. Any changes to the guideline document do *not* need to go through the formal SIG proposal procedures.

If there are any changes to the policy document at:
http://www.apnic.net/docs/policy/add-manage-policy.html

then these do need to go through formal consensus decision making processes.

Therefore you do not need to submit a proposal to change the guidelines. If the guidelines are agreed to by the working group, then the outcome of the guidelines need to be conveyed to the SIG as an informational item.

The guidelines document is intended to be free of policy and to focus on providing additional supporting information that explains the policies and request processes. In the past people have found it helpful to have the information regarding the forms in the guideline document but this may not always be the case.

My name is Yi Lee and I'm the chair of this working gorup. I finally can participate the discussion after fixing the mailing list problem. I try to catch up with the discussion here by digging into the mailing list rchive, please bear with me if I miss any thing already discussed.

I don't think it's appropriate that the form (containing the information about headend, homepass, capacity, etc) should go into the guide line.
Because
1. the guide line should by policy-oriented but not "operation-oriented".
2. leaving the items of the form to APNIC can reach maximum flexibility because I believe it should be more efficient if it's up to APNIC to decide what kind of information needed to justify the address allocation needs. On the other hand, if the form is put into the guide line, once you want to modify any of the items in the form in the future, you have to follow the guide line revision process, i.e. setting up the working group and mailing list, submitting revision proposal to policy sig and voting. The whole porcess can be very time-consumed.
